The Rural Care at Home Accelerator helps rural health partners and care agencies strengthen the systems that allow older adults, people with disabilities, medically fragile residents, and Medicaid members to receive safe, coordinated care at home and in community-based settings.

The program supports local and statewide partners with care access assessments, agency readiness planning, discharge-to-home workflows, referral coordination, technology readiness, emergency preparedness, and sustainability planning.
