A patient- and member-centered technical assistance program for rural home-based care access, agency readiness, and care coordination.
Home-based care works only when the systems behind it are strong enough to hold.
The Rural Care at Home Accelerator helps rural health partners and care agencies strengthen the systems that allow older adults, people with disabilities, medically fragile residents, and Medicaid members to receive safe, coordinated care at home and in community-based settings.
The program supports local and statewide partners with care access assessments, agency readiness planning, discharge-to-home workflows, referral coordination, technology readiness, emergency preparedness, and sustainability planning.
Capitol Care Strategies is positioned to support RHTP-funded partners through non-lobbying technical assistance, program design, implementation support, workforce strategy, care coordination planning, and sustainability development.
